Diffuse adenomyosis with bilateral ovarian dermoid cyst

Presentation

Metrorrhagia.

Patient Data

Age: 45 years
Gender: Female

Moderately enlarged uterus with thickening of the junctional zone (17 mm) of intermediate signal intensity on T1, and homogeneous low signal intensity on T2, in keeping with a diffuse adenomyosis with a uterine leiomyoma (Figo 5).

Bilateral well-defined ovarian masses (the largest on the right) of complex signal intensity. The main component is a fatty signal attenuated on fat-suppressed sequences with an internal nodule on the right "Rokitansky nodule". No significant enhancement was seen on the postcontrast sequences.

MRI features most consistent with diffuse adenomyosis with bilateral ovarian dermoid cysts and uterine leiomyoma.
