Diffuse adenomyosis with bilateral ovarian dermoid cyst

Presentation

Metrorrhagia.

Patient Data

Age: 40 years
Gender: Female

Bilateral well-defined ovarian masses (one on the right and two on the left) of complex signal intensity. The main component is of fatty signal attenuated on fat-suppressed sequences with a chemical shift artifact on T2. No enhancement seen on the postcontrast sequences.

Moderately enlarged uterus with thickening of the junctional zone (18 mm) of intermediate signal intensity on T1, and homogeneous low signal intensity on T2, in keeping with a diffuse adenomyosis

Case Discussion

MRI features of bilateral ovarian masses of complex signal with attenuation of the fatty component on T1 fat sat sequences, most consistent with dermoid cysts in a patient with diffuse adenomyosis.
