Diffuse idiopathic skeletal hyperostosis and ossification of the posterior longitudinal ligament

Patient Data

Age: 75 years
Gender: Male

Multiple flowing osteophytes anteriorly between C2 and T3 in keeping with diffuse idiopathic skeletal hyperostosis (DISH).

Ossification of the posterior longitudinal ligament (OPLL), particularly marked between C2 and C5. This causes moderate to severe stenosis of the central canal between C2 and C5.

Moderate degenerative changes.

Linear soft tissue calcification noted posterior to C3-C5 suggestive of nuchal ligament ossification.

Case Discussion

Diffuse idiopathic skeletal hyperostosis is a systemic bone-forming condition characterized by the presence of at least three bony bridges at the anterolateral spine commonly associated with ossification of the posterior longitudinal ligament.
