Diffuse large B-cell lymphoma - fibula

Diagnosis almost certain

Presentation

Painful right leg. X-rays (not available) demonstrate a lytic lesion of the fibular diaphysis.

Patient Data

Age: 70 years
Gender: Female

A bony lesion at the junction middle-distal 1/3 of the fibular diaphysis. It elicits a low signal on T1, an intermediate signal on T2, and a high signal on STIR with restricted diffusion. The postcontrast sequences show a heterogeneous enhancement. There is a cortical breach with extension to the adjacent soft tissues (fibularis longus and extensor hallucis longus).

Case Discussion

MRI features of a fibular bony lesion with cortical rupture and extension to the adjacent soft tissues, pathologically proven as diffuse large B-cell lymphoma (DLBCL).

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma.
