Case contributed by Mohammad Taghi Niknejad
Diagnosis almost certain

Presentation

Work up for abdominal pain and chronic constipation.

Patient Data

Age: 35 years
Gender: Male

Marked circumferential increased wall thickness without luminal obstruction is present at the caecum and ascending colon, measured about 17 cm in length. Surrounding fat stranding and several enlarged regional lymph nodes with SAD less than 15 mm are also evident. There is no sign of local invasion and no detectable metastasis.

Case Discussion

The patient underwent a right hemicolectomy, and histopathology evaluation confirmed diffuse large B-cell lymphoma of the colon, a very rare tumour.

On imaging, compared to colorectal adenocarcinoma, lymphoma affects longer and multiple colonic segments, characterised mainly by circumferential wall thickening.
