Presentation

Progressive high dysphagia to solids ongoing for 5 months.

Patient Data

Age: 70 years
Gender: Male
Barium

Upper GI barium study reveals the corkscrew appearance of the esophagus, particularly at the distal segment with uncoordinated tertiary contractions interrupting the normal peristalsis, resulting in a delay in barium transit.

An incidental small hiatus hernia is seen as well.

Case Discussion

Corkscrew appearance, particularly of the distal esophagus is classical, although uncommon finding in diffuse esophageal spasm.

The lower esophageal sphincter, unlike in achalasia, will relax normally.

Pseudodiverticula may be seen.
