Female patient known with systemic lupus erythematosus on treatment. Now presented with deteriorating respiratory function. An HRCT chest was requested.

Diffuse splenic calcification - systemic lupus erythematosus

An incidental finding of numerous calcifications in a tightly grouped distribution are demonstrated throughout the spleen. Note the spared peripheral rim of splenic parenchyma and capsule.
The patient did have features of collagen vascular disease associated ILD on chest imaging (not shown).
Case Discussion
Diffuse splenic calcification is a rare manifestation of systemic lupus erythematosus (SLE) and the distinctive calcification pattern can hint at the diagnosis. Splenic calcification secondary to granulomatous disease should be excluded in endemic areas.
