Diffuse uterine adenomyosis - endometrial hyperplasia

Diagnosis probable

Presentation

Perimenopausal metrorrhagia.

Patient Data

Age: 55 years
Gender: Female

The MRI sequences demonstrate an enlarged uterus with diffuse thickening of the junctional zone endometrium-myometrium ranging from 10 to 35 mm, displaying an intermediate signal on T1, and low signal on T2 with numerous foci of high signal indicating menstrual hemorrhage into the ectopic endometrial tissues.

Diffuse and relatively regular thickening of the endometrium with no restricted diffusion on DWI/ADC indicating probably associated endometrial hyperplasia. 

A small right ovarian follicle is noted (20 mm) with minimal effusion in Douglas pouch.

Case Discussion

The MRI features are suggestive of diffuse uterine adenomyosis with associated endometrial hyperplasia. This association was already reported in the literature 1.
