Presentation

Shortness of breath.

Patient Data

Age: 30 years.
Gender: Male

Globally enlarged cardiac silhouette, lungs clear, no pleural effusion.

Enlargement of all 4 cardiac chambers, with most severe involvement of the left ventricle. Mild left basilar atelectasis. Contrast reflux into a dilated IVC. Ascites.

Case Discussion

The CT shows a markedly enlarged left ventricle. The echocardiogram (not shown) demonstrated an enlarged thin-walled hypokinetic left ventricle with an ejection fraction of <25%. Contrast reflux into a dilated IVC and ascites imply that the right heart is dysfunctional, which was confirmed on the echocardiogram. The etiology of this patient's cardiomyopathy is unknown.
