There is a right well-circumscribed fluid-filled cyst of CSF signal on all pulse sequences with no enhancement or surrounding gliosis. A small vessel traversing the lesion is noted, well-visualized on the postcontrast sequence as well as the 3D-TOF (source images).
Case Discussion
MRI features typical of enlarged perivascular spaces (incidental finding in this case) which are a common finding and should not be mistaken for other CNS lesions (see differential diagnosis).
