Presentation

Low back pain radiating to the right lower limb.

Patient Data

Age: 40 years
Gender: Female
mri

L4-L5 disc degeneration with a central disc herniation extending above the endplates. A small detached disc fragment right posterosuperior to the disc herniation, compressing mainly the adjacent L4 nerve root. It elicits an isosignal to the intervertebral disc on T1, slight high signal on T2, and STIR with peripheral enhancement on postcontrast sequences, suggestive of a free disc fragment that has no continuity with the native L4-L5 disc.
