Disc sequestration

Case contributed by Ammar Haouimi

Low back pain radiating to the right lower limb.

Patient Data

Age: 40 years
Gender: Male

Small detached disc fragment right posterosuperior to the disc herniation at L5-S1, compressing mainly the adjacent S1 nerve root. It elicits an isosignal to the intervertebral disc on T1, slight high signal on T2, and STIR with peripheral enhancement on postcontrast sequences, suggestive of a free disc fragment that has no continuity with the native L5-S1 disc.

Small right subarticular disc herniation at L2-L3.

Circumferential disc bulge at L4-L5.

Case Discussion

MRI features most consistent with L5-S1 disc sequestration.
