Large cyst apparently arising from the disc and not the facet joint or nerve sheath. Note minor pressure erosion on the posterior vertebra. It also compresses and distorts the adjacent proximal L5 nerve root sheath causing sciatica.
Case Discussion
Etiology of these very rare cysts is uncertain but probably related to an annular fissure
Differential diagnosis includes synovial cyst, ganglion cyst, perineural cyst, extradural arachnoid cyst, and neuroma with cystic changes.
