The C6/7 disc space is fluid filled and the anterior and posterior annulus deficient. Extensive epidural phlegmon compresses the cord with resultant high T2 signal. A collection is seen anterior to the spine, but only tiny pockets of fluid are seen in the epidural space.

This patient had a known history of recent intravenous drug use. He underwent a biopsy with the following microbiology.
