Presentation

Chronically ill man.

Patient Data

Age: 25
Gender: Male
X-ray

Multiple miliary lung nodules.

Right paravertebral soft tissue swelling suggesting spondylodiscitis +/- right psoas abscess.

CT

Bilateral <5 mm miliary nodules with a basal predominance.

CT

Multiple enhancing lesions, not visible on the non-contrast.

CT

Large right psoas abscess. L4 vertebral body destruction.

MRI

Multi-level vertebral body enhancement with sparing of the discs.

Axial images again demonstrate the psoas collection. No epidural components.

Case Discussion

Proven Tuberculosis.
