Distal appendicular metastasis: metatarsal

There is an osteoblastic sunburst periosteal reaction and bony destruction of the 2nd metatarsal midshaft.

Case Discussion

This was a metastasis from a small cell malignancy of lung.

When not due to primary bone tumours, these appearances are typically seen in hypervascular bone metastasis. The primary malignancy is often renal, thyroid and rarely lung. Considering the location, the distal appendicular skeletal metastases are most often seen in advanced lung cancer.
