Distal femoral osteosarcoma with pathological fracture

Diagnosis almost certain

Presentation

Left knee pain

Patient Data

Age: 7 years
Gender: Male
  • lateral and frontal radiographs at initial presentation (1 and 2)
    • aggressive bone lesion in the distal femur
    • wide zone of transition
  • lateral and frontal radiographs 2 weeks later (3 and 4)
    • the lesion has enlarged
    • new pathological fracture
  • lateral and frontal radiographs of fracture on follow up (5 and 6)
    • marked periosteal reaction
    • impaction of the previously imaged pathological fracture

STIR, T1, and T2 with fat saturation demonstrate the extensive soft tissue component, not apparent on the initial presentation radiographs.
