Distal fibular and fifth metatarsal base fractures

Presentation

Inversion injury during sport.

Patient Data

Age: 30 years
Gender: Male

Transverse fracture of the distal fibula, below the level of the talar dome (Weber A). No convincing evidence of syndesmosis injury, with distal tibiofibular overlap measuring approximately 6 mm.

Incomplete extra-articular transverse fracture of the proximal fifth metatarsal.

Otherwise, normal bone and joint alignment. Normal ankle mortise. Lateral ankle soft tissue swelling.

Case Discussion

The base of the 5th metatarsal is a review area to assess on the lateral projection of the ankle as fractures can present with pain and tenderness over the lateral malleolus. This case has a lateral malleolar fracture AND a base of 5th metatarsal fracture.
