Distal iliotibial band insertional inflammation

Presentation

Lateral knee pain.

Patient Data

Age: 55 years
Gender: Female

The distal iliotibial band as it inserts onto Gerdy's tubercle is thickened with peri- and intra-tendinous high signal including mild edema in the adjacent inferolateral Hoffa's fat pad. Mild high signal deep to the iliotibial band at the lateral femoral condyle, which should be considered non-specific, without high signal of the iliotibial band itself nor the superficial soft tissues at this level.

Case Discussion

This is an unusual case of distal iliotibial band syndrome where there is inflammation of the insertional fibers (i.e. an enthesitis) rather than at the level of the lateral femoral condyle, which is the typical site of this syndrome.
