Distal radioulnar joint and triangular fibrocartilage complex (TFCC) injury

Presentation

Fall during a bicycle accident 3 weeks earlier. Positive piano key sign.

Patient Data

Age: 20 years
Gender: Male

Bone bruise and dorsal displacement of the ulnar head of about 10 mm. Effusion in the distal radioulnar joint. Focal bleeding into the ulnar insertion of the discus with avulsion of the ulnar styloid process.

Case Discussion

Marked dorsal displacement of the ulnar head, volar capsular hematoma and effusion of the distal radioulnar joint indicates rupture of the volar radioulnar ligament and distal radioulnar joint instability.
