Diverticulitis complicated by a colovesical fistula

Case contributed by Lam Van Le
Diagnosis certain

Presentation

Lower abdominal pain, recurrent urinary tract infections, and pneumaturia (passing gas in urine).

Patient Data

Age: 75 years
Gender: Male

MPR with zoom and oblique

ct

A diverticulum in the sigmoid colon measuring 37x38 mm, with thickened walls, surrounding fat infiltration, strong enhancement, and an indistinct border with the bladder wall. MPR reconstructed images with magnification and oblique views show a fistula tract from the sigmoid colon diverticulum into the posterior bladder dome area - deviated to the left. There is a fluid-air level within the bladder.

Case Discussion

The imaging findings are consistent with a sigmoid colon diverticulitis complicated by a colovesical fistula. Diverticulitis is the most common cause of this condition, followed by colon cancer and Crohn's disease.
