Three duodenal outpouchings are seen, two are projecting from the medial aspect of the second part of the duodenum and a larger one projecting from the superior wall of the third part of the duodenum. They show smooth wall and containing air and retained intestinal content (food) mottled with air. They vary in size and gas content over the phases of the study due to intestinal peristalsis. On imaging with oral contrast, their content is outlined by the contrast confirming their connection to the duodenal lumen. Features are consistent with uncomplicated duodenal diverticula.
A small right renal cortical cyst is also noted.
Case Discussion
Duodenal diverticula are seen as saccular outpouchings and may contain gas, fluid, contrast, or food debris. They are usually incidentally detected on abdominal CT and mostly asymptomatic. Rarely, they can be complicated with inflammation, hemorrhage or compression of distal CBD with obstructive jaundice (Lemmel syndrome).
