Duodenal intramural haematoma

Case contributed by Dr Ahmed Elowaidy

Presentation

Vague abdominal pain

Patient Data

Age: 5
Gender: Male

US abdomen

Modality: Ultrasound

Ultrasound of the abdomen showed a large midline cystic lesion causing bowel compression with no flow on color Doppler (not shown).

CT abdomen

Contrast enhanced CT scan of the abdomen using IV and oral contrast showed a large intramural filling defect within the 3rd and 4th parts of the duodenum and proximal jejunum, causing luminal attenuation. This filling defect showed lamellated/layering hyper density suggestive of hemorrhage. The appearance is very suggestive for intramural haematoma (for which the underlying cause in this particular cause is undetermined).

Follow up ultrasound 1 month later (not shown) showed resolution of this filling defect
