Duplex left kidney with an ectopic insertion of the ureter draining the upper moiety as well as severe hydronephrosis and hydroureter. 

The lower moiety shows average contrast excretion with moderate fullness of its collecting system and its draining ureter is not dilated.

The left kidney shows double moiety with obstructed upper moiety with dilatation of its ureter along its course & the lower moiety shows mild hydronephrosis.

Bilateral reflux: The left upper pole moiety is only minimally opacified. There is reflux into the left lower moiety which is dilated. The right system is not dilated.
