Duplex kidney with bifid ureters

Presentation

Underlying nasopharyngeal carcinoma. Incidental findings on CT scan done for disease staging.

Patient Data

Age: 65 years
Gender: Male

Incidental findings of left duplex collecting system with two ureters that unite in the pelvis before draining into the urinary bladder. This is best seen in the delayed scan in coronal view (MIP images). No hydronephrosis or hydroureter. No renal or ureteric calculi.

Right kidney is normal and has a single collecting system.

No other significant findings in the rest of the abdomen.

Case Discussion

Duplex kidneys are anatomical variants in which the kidney has two separate pelvicalyceal systems that drains into either a single ureter at the pelviureteric junction (PUJ) or double ureters that either unite before reaching the bladder (bifid ureters) or that drain separately into the bladder (complete duplication). 

When there are two separate ureters, the upper renal moeity ureter has an ectopic insertion and is often associated with a ureterocele, causing urinary obstruction, while the lower renal moeity ureter often inserts less obliquely, causing vesicoureteric reflux (Weigert-Meyer law).
