Dural arteriovenous fistula

Presentation

Headache, loss of consciousness, blurred vision, treated as case of increase intracranial pressure.

Patient Data

Age: 45 years
mri

Severe distended dural venous sinuses in the posterior fossa (sigmoid and transverse sinuses), enlarged, tortuous vessels in subarachnoid spaces, dilated cortical veins, dilated tortuous vascularity around cavernous sinuses, dilated superior ophthalmic veins.

Dilated, tortuous venous varicosities, indenting the brainstem with prominent external carotid supply via numerous scalp vessels.

No intraparenchymal nidus.

Cerebellar tonsillar coning into foramen magnum.

Empty sella, enlarged Meckel cave, intraocular protrusion of the left optic nerve head, coloboma on right side.

Case Discussion

Diffuse, enlarged all dural venous sinuses, and cortical veins, vessels in subarachnoid spaces, central venous drainage, severe venous varicosity mainly in posterior fossa, with prominent external carotid supply via numerous scalp vessels.

Venous congestion and signs of increase intracranial pressure, empty sella, cerebellar coning into foramen magnum.
