Dural arteriovenous fistula - anterior cranial fossa

Presentation

Adult patient presenting with sudden onset headache and decreased conscious state.

Patient Data

Age: adult

Large spontaneous SDH, seen crossing the fronto-parietal suture and extending along the falx. Significant mass effect is present with midline shift.

Subsequent CTA demonstrates abnormal vascular structures extra-axially along the floor of the anterior cranial fossa.

Angiography revealed an anterior cranial fossa DAVF with supply from both ophthalmic arteries via ethmoidal branches. No ECA supply.

Case Discussion

A large cortical vein was identified at craniotomy for evacuation of the hemorrhage.
