Presentation

History of breast cancer. The patient presented with headache and blurred vision.

Age: 55 years
Gender: Female

Thickened dura is noted in axial FLAIR images more pronounced at both frontoparietal regions

On post-contrast study there is irregular nodular homogenously enhancing Dural thickening forming nodular enhancing small extra-axial masses at the interhemispheric fissure.

Case Discussion

Breast cancer is one of the common malignancies that metastasis hematogenously to dura and causing dural metastasis.

Dural metastasis can present on imaging as diffuse dural thickening that shows homogenous enhancement on post-contrast study or it can present as multiple extra-axial dural based homogenously enhancing masses resembling meningiomas.

Dural thickening and dural-based masses in a case of breast cancer is always suspicious of metastasis.

Differential diagnosis include:

  • infective meningitis

  • intracranial hypotension
