Autopsy. Three axial sections through the midbrain (upper section) and pons (lower two sections) demonstrate multiple Duret hemorrhages. The cerebral peduncle on the left of the image is abnormal presumably the result of herniation, either direct compression by a herniated uncus or compression against the free edge of the tentorium by contralateral mass effect (Kernohan phenomenon)

Case Discussion

Duret hemorrhages are small usually multiple hemorrhages in the midbrain or pons of patients who are rapidly developing brain herniation.
