Temporal lobe epilepsy.
Patient Data
A relatively well-defined lobulated right posterior temporal lesion is demonstrated. It is centered in cortical grey matter, in contact with the tentorium cerebelli (measuring 36 x 30 x 20 mm) with involvement of the parahippocampal gyrus and collateral sulcus. It elicits a low signal on T1, high signal "bubbly appearance on T2, and mixed-signal with bright rim sign and partial attenuation of some of the "bubbles on FLAIR sequence. No restricted diffusion is seen on DWI-ADC. No enhancement or surrounding edema is noted.
Case Discussion
The MRI features are suggestive of a dysembryoplastic neuroepithelial tumor (DNET) of the temporal lobe which is considered the most common location (over ~65% of cases).
