Presentation
Longstanding seizures.
Patient Data
The MRI study shows a well-defined cortical based lesion at the left basi-frontal lobe which appear hypointense on T1, hyperintense on T2 with no enhancement on post-contrast study and no diffusion restriction.
The lesion demonstrates pseudocystic “bubbly” appearance on T2 weighted images and it is partially suppressed on FLAIR images showing a characteristic ring-like hyperintense signal (FLAIR bright rim sign).
We note lack of mass effect, no perilesional edema, intralesional calcification or hemorrhage.
Incidental note is made of a left sphenoid sinusitis.
The images illustrate the characteristic "T2 soap-bubble appearance" and "FLAIR bright rim sign" of DNETs.
Case Discussion
The MRI findings are suggestive of dysembryoplastic neuroepithelial tumor (DNET).
