Early axial sliding hiatus hernia (esophagogram)

Diagnosis almost certain

Presentation

Epigastric pain and food stuck in his esophagus two weeks ago. He also lost 22 Ibs over last 6 years.

Patient Data

Age: 55 years
Gender: Male

Esophagogram showing early axial sliding hiatus hernia with a gastric wall extension 24 mm above the gastric folds ( outlet ) reaching B ring level.

B ring diameter is measuring 2 cm excluding Schatzki ring.

Case Discussion

Early detection of sliding hiatus hernia is one of the main advantages of esophagogram. Trendelenburg position is important for early assessment of reflux and sliding hernia.

The presence of gastric wall above the diaphragm level and a distance of greater than 2 cm between gastric folds and B ring are suggestive findings of early hiatus hernia.
