There is a small retroclival bony defect with non-aggressive well-defined margins, associated with a small bony stalk protruding into the prepontine cistern.
Case Discussion
Retroclival ecchordosis physaliphora is a hamartomatous notochordal remnant at the dorsal wall of the clivus, that typically is located intradurally in the prepontine cistern. This congenital benign lesion is sometimes associated with a bony stalk that is pathognomonic for this entity.
Histopathologic differentiation of this lesion from chordoma is difficult, so contrast enhancement and the presence of clinical symptoms are highly reliable parameters in differentiation of these two entities.
