Presentation

Weakness, elevated parathormone level (above 3000 pg/ml, normal 15-65 pg/ml) and blood calcium (4.1 mmol/l, normal 2.2-2.55 mmol/l).

On SPECT images a region of high Tc-99m Sestamibi uptake within right hemithorax is seen.

There is a big calcified mass at the right anterior superior mediastinum with a vivid Tc-99m MIBI uptake.

There are multiple bone lytic masses in a background of heterogeneous increased bone attenuation.

Case Discussion

This is an unusual finding in a patient with long history of hyperparathyroidism - ectopic parathyroid tumour in the right superior prevascular mediastinum.

The lytic bone lesions are thought to be brown tumours.

Heterogeneous increased bone attenuation has not been explained.
