Elongated lower pole of the patella

The lower pole of the patella is elongated with a smooth border and contains a fine transverse lucent line suspected of fracture.
In addition, mild degenerative changes are seen as subchondral sclerosis and tibial intercondylar spine spiking.
Case Discussion
As seen in this case, the smooth elongation of the patella's lower pole is rare and seems to be an anatomical variant. Although a long-standing Sinding-Larsen-Johansson disease could be considered in the differential diagnosis, particularly if accompanied by cortical irregularity and fragmentation.
