Embedded intrauterine contraceptive device

Presentation

Missing IUD thread. No pain. Previous two LSCS.

Patient Data

Age: 35 years
Gender: Female

There is an abnormally oriented IUD in the pelvis. Its stem is directed towards the right hip. There is no other radio-opacity in the KUB region. Bones reveal no abnormality.

UTERUS shows normal size without myometrial mass lesions. There is an anechoic diverticulum (8 x 5 x 4 mm) from the anterior wall at the previous LSCS scar region. The combined endometrial thickness is 4 mm. There is no endometrial collection or lesion. There are a few cervical nabothian cysts.

The IUCD is seen as an echogenic line/ dot with acoustic shadow depending on the long-axis or the short-axis image. IUCD stem is in the right lateral wall myometrium at the uterine body-cervix junction region. There is no protrusion from serosa. The other end of IUCD is in the endometrial canal at the LSCS scar region.

The right ovary shows a thin-walled cyst with low-level internal echoes (24 x 20 x 18 mm - 4.8 cc). The left ovary shows a similar cyst (5 x 13 x 7 mm - 0.76 cc).

There is no extra-ovarian adnexal mass lesion.

Case Discussion

The case shows an embedded intrauterine contraceptive device. There are two additional findings in the case which are the uterine cesarean section scar diverticulum and bilateral ovarian endometriomas.
