Presentation

Right flank pain, fever and leukocytosis.

Patient Data

Age: 35 years
Gender: Female
x-ray

Extensive gas overlying the renal parenchyma, collecting system, and perinephric tissues on the right side. Additional linear gas shadows along the paraspinal region, representing retroperitoneal air.

Case Discussion

The findings of the abdominal x-ray are compatible with emphysematous pyelonephritis and retroperitoneal free gas.
