Emphysematous pyelonephritis

Presentation

Abdominal pain and distension. Known case of diabetes mellitus type II.

Patient Data

Age: 60 years
Gender: Female

Enlarged left kidney with almost completely destroyed renal parenchyma and distorted architecture. Numerous linear streaks of gas are seen within the renal parenchyma. 
Perinephric fat stranding is noted with small bubbles of air present in the retroperitoneum.
No extension of gas to the left ureter or abnormal dilatation.
Left renal vessels are intact.
No gross pneumoperitoneum.
Imaged lung bases show mild bilateral pleural effusions with underlying basal atelectasis.

Case Discussion

Enlarged left kidney with almost completely destroyed renal parenchyma and distorted architecture and internal numerous linear streaks of gas with mild perinephric fat stranding. Features are of left side emphysematous pyelonephritis (type I).
