A large well-demarcated pleural fluid collection with foci of rim calcification is seen in the upper part of the right hemithorax related to the right upper lobe with extension through the chest wall into an extrathoracic compartment. There is associated bony erosion of the adjacent ribs especially the third rib.

Empyema necessitans refers to extension of a pleural empyema out of the pleural space and into the neighboring chest wall and surrounding soft tissues.
