En plaque meningioma right sphenoid

Diagnosis probable

Presentation

2 month history of headache/dull pain around the right eye with proptosis and diplopia.

Patient Data

Age: 20 years
Gender: Female

Infiltration, destruction and expansion of the right sphenoid (predominantly greater wing but also lesser wing) with periosteal new bone extending into the orbit.

Heterogeneous (relatively low signal intensity) mass centered on the greater wing of sphenoid with diffuse (vivid) enhancement extending anteriorly into the orbit (compressing the lateral rectus) and posteriorly into the middle cranial fossa.

Case Discussion

Appearances are typical of en plaque meningioma arising from dura overlying the greater wing of sphenoid infiltrating into and through bone, entering the orbit.
