Encysted perifissural hydropneumothorax

Case contributed by Ian Bickle
Diagnosis certain

Presentation

Right sided pneumothorax. Follow up.

Patient Data

Age: 60 years
Gender: Male

Hyperinflated lungs.

Small hydropneumothorax.

7cm elliptical part fluid-gas filled opacity in the left mid zone.

Case Discussion

Rarely encysted pleural effusions are identified in the fissures. This is the first time I have had a case of an encysted perifissural hydropneumothorax.

Air and fluid have equal rights to track into the fissure. It's a pretty unusual (and cool) appearance, which if one approaches logically and with first principles makes sense.

A good case to try on friends, especially those prior to a VIVA exam.
