Endobronchial malignancy with air trapping

Diagnosis probable

Presentation

Progressive shortness of breath over 1 month.

Patient Data

Age: 70 years
Gender: Male

No PE seen.

There was mediastinal adenopathy noted with a right main bronchus lower lobe endobronchial lesion.  On the expiratory CT examination significant air-trapping in the right hemithorax was seen with a degree of mediastinal shift

Case Discussion

This case demonstrates an endobronchial malignancy with air trapping.  The malignancy was thought to be secondary to metastasis of the patient's known renal cell carcinoma, or a simultaneous primary lung malignancy.  However a conservative approach to treatment was taken and so further investigation was not performed.
