Endometrial carcinoma

Presentation

Vaginal bleeding.

Patient Data

Age: 50 years
Gender: Female
mri

Large soft tissue mass with central necrosis within the endometrial cavity. It elicits an iso to low signal to the myometrium on T1, iso to high signal peripherally on T2 with central necrosis of low signal and restricted diffusion mainly of the peripheral soft tissue component. The postcontrast sequences demonstrate a heterogeneous enhancement of the peripheral solid component with irregular inner margin. Invasion the the deep myometrium as well as the serosa at the posterior aspect of the uterine body well-demonstrated on postcontrat sequences.

Moderate ascites with thickening and enhancement of the peritoneal reflections (peritoneal carcinomatosis). Small pelvic lymphadenopathy are also noted.

Case Discussion

The MRI features are suggestive of endometrial tumor (pathologically proven as carcinoma) with peritoneal carcinomatosis. Stage IVb according to the revised 2009 FIGO staging.
