Pelvic pain, raised Ca-125 levels.

Patient Data

Age: 35 years
Gender: Female

Cystic lesion, measuring approximately 60 x 51 x 63 mm in the left adnexa, appearing hyperintense on T1W and heterogeneously hypointense on T2W images with evidence of T2 shading sign. Ovarian follicles are seen at the periphery. Lesion is indenting the uterus medially, external iliac vessels laterally, sigmoid posteromedially and urinary bladder anteromedially.

Case Discussion

Findings typical of an endometrioma. Endometriomas can also lead to raised CA-125 levels. Care should be taken not to mistake them for malignant lesions.
