Case contributed by Jeremy Jones
Diagnosis certain

Presentation

Leg pain and weakness. Tender calves. Dense soft tissues on examination. White cell count elevation.

Patient Data

Age: 13 years
Gender: Female
mri

Bilateral and symmetric thickening with T2 hyperintensity of the fascia throughout both legs which is most pronounced in the calves.

Mild muscle edema suggests inflammatory change. Soft tissue hyperintensity and thickening. No significant effusion or lymphadenopathy.

Case Discussion

Gross fascial thickening with soft tissue thickening and a raised white cell count is suggestive of eosinophilic fasciitis.  The differential white cell count revealed an eosinophilia which clinches the diagnosis here.
