Presentation

Scalp swelling in the left parietal region. No pain or fever.

Patient Data

Age: 20 years
Gender: Male
x-ray

A solitary punched out lytic lesion without sclerotic rim in the left parietal region.

Case Discussion

Eosinophilic granuloma is common among the young. A punched out lytic lesion without sclerotic rim is a typical appearance, as is a hole within a hole appearance which is due to bevelled edge in the skull defect, with the inner table defect is larger than the relatively small outer table defect.

Differentials to consider, particularly in older individuals are solitary plasmacytoma and metastasis.
