Epidermal inclusion cyst (gluteal region)

Presentation

Right gluteal region painless swelling for the last few months.

Patient Data

Age: 35 years
Gender: Female

Well defined, thin walled lesion in the region of the interest, measuring about 30 x 28 x 21 mm. It is located in the subcutaneous plane and abuts the skin. There is a tiny hypoechoic track from the lesion to the overlying skin. The lesion is separate from muscle/bone. It shows a heterogeneous hypoechoic echo pattern without calcification/cystic changes/ vascularity. There is acoustic enhancement. There is no perilesional fat inflammation or hypervascularity.

Case Discussion

Ultrasound shows a thin walled, avascular lesion in the subcutaneous plane with a punctum which is a hypoechoic track from the lesion to the skin. Findings favor an unruptured epidermal inclusion cyst.

The surgical excision of the lesion was undertaken. Histopathology confirmed the lesion being epidermal inclusion cyst.
