Case contributed by Dr Maulik S Patel

Swelling in nape of neck - about 6 - 7 yrs. No pain. Gradual increase in size.

Patient Data

Age: about 60 yrs.
Gender: Male

There is a mid line soft tissue lesion in nape of neck. It is in subcutaneous plane and abutts skin .  Lesion measures about 70 x 40 x 30 mm. It is encapsulated. Lesion shows relatively homogeneous echopattern with few linear tiny anechoic cleft like foci. Lesion is avascular showing dopplar artifact at very high PRF.  There is presence of posterior acoustic enhancement. With a stand off jel pad, there is a hypoechoic tract from lesion to skin.

Case Discussion

Ultrasound findings very much favour epidermal inclusion cyst; showing a tract from lesion to skin.

Histopathology follow up is not available.
