Epidermal inclusion cyst - scapular region

Diagnosis almost certain

Presentation

Right scapular region swelling for 6-8 months. Local pain for 1 week prior to presentation.

Patient Data

Age: 45 years
Gender: Male

There is an encapsulated, compressible, hypoechoic lesion abutting skin of scapular region. There is a hypoechoic tract from lesion to skin. At deeper aspect of the lesion; there is a hypoechoic tract from lesion to surrounding subcutaneous fat. There is no intra-lesional flow signal. However, there is perilesional flow.

Case Discussion

Male patient presented with scapular region swelling during last few months. Lesion becomes painful during last few days. Ultrasound shows lesion with a punctum - diagnostic point for epidermal inclusion cyst. There is leak from lesion which explains pain.

Surgical excision was performed. Leak was present from lesion.

Histopathology revealed epidermal inclusion cyst.
