Epidermoid cyst

Case contributed by Dr Bita Abbasi

Presentation

Headache

Patient Data

Age: 42
Gender: Male
Modality: MRI

There is a cystic lesion in the right CP angle with restricted diffusion and rim enhancement. The findings are characteristic for epidremoid cyst.

Case Discussion

Intracranial epidermoid cysts are the third common mass lesions in the CPA after acoustic schwannoma and meningioma. A helpful imaging finding in epidermoid cyst is restricted diffusion in DWI/ADC images that is a useful clue to differentiate the lesion from arachnoid cyst. Enhancement is not seen after contrast adminstration, but peripheral wall enhancement is seen in some instances (as in this case). Note that a pronounced enhancement should prompt malignant degeneration.
