There is a T2 hyperintense lesion without complete FLAIR suppression in the right cerebellopontine angle with restricted diffusion and rim enhancement.

Case Discussion

The findings are characteristic for epidermoid cyst. Intracranial epidermoid cysts are the third common mass lesions in the CPA after acoustic schwannoma and meningioma. A helpful imaging finding in epidermoid cyst is restricted diffusion in DWI/ADC images that is a useful clue to differentiate the lesion from an arachnoid cyst. Enhancement is not seen after contrast administration, but peripheral wall enhancement is seen in some instances (as in this case). Note that a pronounced enhancement should prompt malignant degeneration.
